How To Make An Appointment

There are two ways to make an appointment:

  1. By telephone.  Please telephone (0114) 2510000 to book an appointment with a member of our Reception Team. Our telephone lines are open from 08:00 until 18:30.
  2. In person. You can walk into the Practice to speak with a member of our Reception Team from 08:00 until 18:30.  The exception to this is when we close one Wednesday afternoon each month for compulsory staff training (known as QUEST).
Please note our Reception Team do not simply answer the telephone, they also complete a number of other tasks such as processing samples.  Our Team are specially trained to help you get the most appropriate treatment as fast as possible.  In order to do this, they need to ask questions about why you need to make an appointment so you can see the most appropriate clinician for your medical need.

Appointment

Not a Walk-In Centre

We provide an appointment-only service thereby unable to provide a walk-in service for patients.  We do not have an x-ray machine on-site so are unable to deal with any potential breakages due to slips, trips or falls.

How To Cancel an Appointment

It is important to contact a member of our Reception Team as soon as you know you are unable to attend your scheduled appointment.  This will allow your appointment to become available to another patient.  Please telephone (0114) 2510000 and select option 1.

Patients who persistently fail to notify the Practice of their non-attendance may ultimately be removed from the Practice's patient register.

Late For Your Appointment

Please attend your appointment on time as, if you are late, you will not be seen by the clinician later the same day.   Instead you will need to make a new appointment with a member of our Reception Team.

Chaperones

It is the policy of this Practice to respect the privacy and dignity of our patients.

If you would like a chaperone to be present during a physical examination by a clinician, or if you would prefer to be examined by a clinician of the same sex as yourself, please inform a member of our Reception Team at the time you book your appointment.

Interpreter

If you require an interpreter to attend your appointment with you, please bring a member of your family or a friend.  Alternatively, please inform a member of our Reception Team prior to your appointment so an interpreter can be arranged for you in advance.

Text Reminder Service

We have a texting service which allows you to receive confirmation and reminders about your appointments.  You will be asked if you would like a text reminder by a member of our Reception Team when making an appointment.

Please remember to update your contact details with us whenever you change your home address, home or mobile telephone numbers, or email address.

Home Visits

Home visits are made for those patients who, because of their medical condition, are unable to attend the Practice in-person. If you would like to discuss whether a visit is necessary please telephone our Reception Team before 10:00 on (0114) 2510000.

Late requests will mean the visit may not be made until the next working day. Please remember that several patients can be seen at the Practice in the same time it takes to do one home visit.

Community Transport

If you need transport to attend a Healthcare only appointment (for example, at a dentist/GP/hospital) which is booked Monday-Friday, 9.00am-5.00pm, community transport is available for you to book directly to take you to, and collect you from, your Healthcare appointment.

Active Response is our community transport provider and can be contacted by telephoning (01709) 323123, Monday-Friday, 9.00am-3.00pm.  You must book at least 24 hours in advance of your Healthcare appointment.

The costs are subsidised by Derbyshire County Council and NHS England and we understand they are approximately a third cheaper than using a taxi service.

Minibus

Private Medical Examinations

Medical examinations for special purposes - such as DVLA/HGV fit-to-drive medicals, or fit-to-adopt/foster medicals - will require longer than a routine appointment.

These examinations do not form part of NHS Services and you will be charged an appropriate fee.  When making your appointment with a member of our Reception Team they will be able to inform you how much the medical examination will cost.
