How do I register?
We are currently accepting new patients. We can only accept new patients who live in our Practice catchment area, which serves the village of Killamarsh.

You can register by downloading a registration form and a new patient registration form, and handing it in at reception or alternatively you can ask for a new registration form at reception.

How do I change my personal details?
Please inform the Receptionist if you change your name, address or telephone number. We must have your telephone number even if it is ex-directory so we can contact you in an emergency. It will be kept strictly confidential.

How do I make an appointment?
You can book an appointment with a doctor online or you can telephone the surgery. To book online you need to register beforehand, see our online page

How do I book a home visit?
Home visits are made for those patients who, because of their condition, cannot get to the surgery. If you would like to discuss whether a visit is necessary, please telephone the reception staff before 10 am on 0114 2510000.

Late requests will mean the visit may not be made until the next day. Please remember that several patients can be seen at the surgery in the time it takes to do a home visit.

How do I request a list of travel vaccines I require before going on holiday?
Please let our reception team know where you are going on holiday and one of our nurses will phone you back to discuss what you require.

How long does it take for my test results to come back?
Please allow one week for your test results to come back unless the clinician specifies differently. Xrays usually take two weeks

How do I order a prescription?
You can obtain repeat prescriptions in the following ways:

  • Use the tear off section of your previous prescription and hand it to a Receptionist or post it in our prescription box in the surgery
  • Telephone the surgery after 11.30 am and order it from the Receptionist
  • By post to the surgery, again using your tear off slip
  • Direct from the pharmacy - call in at the pharmacy of your choice to arrange for them to collect your repeat prescription for you and get it ready
  • Online yourself - please ask at reception for details

Electronic Prescription Service
You can now have your prescription sent electronically (over the internet securely) to the Pharmacy of your choice.

How do I get a sick note?
You will need to see a GP for your first sick note and for subsequent sick notes you can phone reception

What should I do if I have a dental problem including toothache?
GPs are not trained to provide dental care.

Yes we can! We have a Wellbeing advisor who can sign post to the services that are available

Where can I get information and leaflets about health conditions? has a huge database of leaflets as well as discussion forums where patients can chat, get support and share advice about many medical conditions
NHS Choices has an excellent A-Z of information about symptoms, conditions, medicines and treatments as well as sections on healthy living, health news and local services

Can the practice accommodate requests for attachments such as Work Experience?
We regret that, for reasons of patient confidentiality, the Practice has a firm policy of not allowing individual’s access to the Practice in this way.
