Send a Review to Our Clinicians
Annual Reviews are a way of letting your Practice know how you are managing with your long-term condition, your contraception, or your medication. Only complete an Annual Review if you have been asked to do so by a clinician in our Practice.
Killamarsh Medical Practice has introduced a new process for reviewing a patient's long-term condition annually. The Review is undertaken by completing an online questionnaire, which will be sent directly to the Practice and will be reviewed by one of our Practice Nurses or GPs. A Practice nurse or GP will only contact you should they have any queries or wish to see you for a face-to-face appointment.

Contraception
Your annual Contraception Review is a great opportunity for you to review your contraception choice going forward. Did you know that long acting reversible contraceptives such as the coil or implant are more than 99% effective (more so than the pill)? If you would like to discuss your options further with a clinician, please contact our Reception Team.
Otherwise please complete the online questionnaire to continue your pill. If you are on the combined pill you will need to submit your blood pressure, height and weight reading.
Post Covid-19 pandemic, we are recommending that patients purchase their own blood pressure (BP) machine to enable them to take regular home BP readings. Alternatively, the Practice has a limited number of blood pressure machines which can be borrowed for a maximum of one week.
Hypertension (High Blood Pressure)
If you are being treated for high blood pressure, you can use the online form below to submit your readings to your Practice. Please have at least three readings ready.
